How does the tool work?
This online questionnaire is presented in a range of different languages. Additional support is provided by audio buttons that play an oral version of each question. The tool has been designed to minimise the amount of typed responses, so most questions only require the user to select from a list of options. Space is sometimes provided for the user to type in extra information. This tool does not store any information and therefore must be completed fully in one sitting. Upon completion, a formatted report will be displayed, and you may print or save the report if you wish for your own records. Once the online form has been closed, all data is deleted from the site.
Using the tool
- Where possible, ensure that at least one parent/carer is involved in the completion of the online questionnaire as well as the learner. This is particularly important for young learners.
- Provide an adult practitioner to help with the completion of the questionnaire.
- Some questions must be answered in order to generate the report, e.g. DOB. These mandatory questions are identified with asterisks.
- The system does not store any information, and as such, users will need to complete the survey in a single visit.
